What are Good Sources of Protein (and Carbs and Fat)?

As a personal trainer, this is one of the most frequent questions I am asked. Literally everyone has a protein deficiency in their diets, and increasing it requires more thought than "eat more chicken breast" or "have a protein shake". So I've compiled a thorough, if not exhaustive, list of some great protein sources, as well as carbohydrates and fats, and where they overlap.
